XBOX 360 4 gig console -hitching and freezing

Hi, I am experiencing some issues with the walking dead season 1- plus 400 days CD on my XBOX 360. The game is freezing and hitching when playing from the CD . I know you have already a provision for this situation, and I emailed the redemption form with all the required information, however I have not received any replay or acknowledgement of receipt of the information.

Could you please be so kind to let me know, what is the next step?
