Everyone has a backstory, even if you don't tell it. Invent one. Why is he (I'll assume male here) a sick beggar? Is the sickness his fault? Has he tried to work, or is he just lazy? Has he given up on society? Does he hate society? Is he Batman in disguise?
I've been asked to write a monologue for a character who is a sick beggar.
I wanna ask some ideas of what the monologue should contain as its a blank character with no backstory whatsoever.

This really depends on your goal of what you want to create.
The way I would do it is first think of a day in the modern life of this guy. He asks someone for spare change and gets spat upon. If he's the type that gets mad, he starts trying to go after the spitter, but being sick, he really doesn't stand a chance, the police get called or see this, and he gets thrown in jail. Or, if he's the type that just accepts being treated badly, he sulks back on the street corner, dirty, sick, and broke. You could pick something else instead, maybe even something exceptionally good instead of bad. What sort of things would happen to a sick beggar?
Then, the monologue could start with that, to draw the reader/listener in. The beggar would relate what happened and where he ended up, then express resignation at his current life, and flash back to when the sickness first took hold and what happened then. That puts the beggar in the position of telling a story and what he thinks about things, the way they were and the way they are now.
