He probably just slipped out in the chaos. Plus he was never actually committed so who knows why he was there anyway.
Well, they had no record of him in Arkham Asylum, so if he managed to slip past the guards, they wouldn't notice someone was missing. More importantly, how did Bruce do all the linking in the billiard room of his house and see all the holograms without his cowl? lol.
I've been wondering that as well.
Pretty bad oversight from Telltale, to be honest.
Bruce/Batman is a good detective and it wasn't that complex a crime scene. I think some developers in the forum answered that the visuals was just for the players to follow his investigations.

If you played the Cold Cold Heart DLC and add on challenge maps for Arkham Origins then he did have something like detective vision when not dressed up as Batman but I forget what it was called atm.

Something worth pointing out, on the news they say all of the inmates were returned to their cells. But we know that's not true, because they had no actual record for "John Doe" being there in the first place they had no way of knowing he left.
He left during the chaos and changed his identity once again.
Bruce mentally envisioned the scene were Alfred was kidnapped.

He most likely escaped during the riot in Episode 5.
How do you know he's outside Arkham? Maybe asylum has a bar, part of social reintegration program.
