How do I get the form back?

Hi everybody,

This is my first post :) I absolutely LOVE the Sam&Max game, but right now I must be tired, because I have been stuck for hours!

I have been psychoanalyzed, and declared "insane". The forum is sitting on Sybil's desk, but how on earth do I get it back? I have been wandering around for hours, interacting with just about everything, and I really need a hint how to get the form back so that I can insert it in the right slot....

[EDIT] Never mind, I think I have the answer... I need ALL THREE tests right? So far I have only done the first one! Wheeeeeeeeeeeee it's so nice to figure things out :D

    That's right, you have to pass (fail?) all three tests before she'll give back the form.
