Stuck with Sam in hell!

I'm stuck with Sam in his own personal hell, and despite countless amount of time trying different tactics, I can't make any difference. If anyone has gotten Sam out of hell already, please give me a helpful hint.

    back before he was stuck inside his own, what was it that Sam used to get himself into other people's personal hells?
    maybe you've got a man on the outside?
