I got 12 out of 13. Tried darn near anything but still got one left :mad:.
Anyway, I recommend going through any dialogue option and showing him any item you come across. Here are a few specifics (although I don't remember all of them)
Show him the pillow near the beginning, Show him all of the 4 Homsar relics when he's at the Homsar reservation, Talk to him when he's at Marzistan, talk to him after you've beaten the game...
Not Neccessarily... I have 11/13 insults and there is at least 3 things mentioned above that I did not do during normal game play. I have a funny suspicion that it still may be possible...
I just replayed the game, and I got 10 Strong Sad insults. Among the things I did were
making him open the door while I looked out the window, having every possible conversation with him in his room while under house arrest, having every possible conversation with him in his room after the secession, having every possible conversation with him while at the reservation, talking to him while in Marzistar, having every possible conversation with him in the King of Town's castle, presenting the pillow while in his room, presenting the fake sword while in his room, presenting the pilotwings while in his room, and presenting four Homsar artifacts while at the reservation.
Not sure which of these was a specific trigger, but it should help...hopefully.
Bout half way through the game again and already up to 11, probley will get all 13 by the end of it (still got the entire strong bad the king and extended play stuff)
Key is to listen for strong bad to literally call strong sad a name, make sure u see all of the conversations that u can just from talking to strong sad and i can confirm the following do count.
Showing him the pillow while on house arrest
Showing him the wings before giving it to the cheat
Showing him the tuning fork (i think its either that or the trapper keeper) while hes with u at the homsar territory
I think this is about my only problem with parts of these games; the point-of-no-return bits where you can't get 100% completion because you didn't do an obscure thing at a certain point. That's about my only criticism of these games, I'd say...
And yes, you have to get all of Strong Sad's nicknames on your first playthrough; you can't go back in extended play to pick up ones you missed. For what it's worth, I normally hate that in videogames, but in this case there were topics of conversation with Strong Sad that it just didn't make sense to go back and repeat later in the story.

3 - Look at Strong Sad's Bed
4 - Give Pillow to Strong Sad
5 - Go through all the conversation options with Strong Sad
5-7 - Go through all convo options with Strong Sad at Bleak House
8 - Show Pilotwings Medal to Strong Sad before giving it to The Cheat
- Use the Fake Sword on Strong Sad at Bleak House
9 - Talk to Strong Sad after Surrender
10 - Show Strong Sad the Tuning Fork when you are at Homesar's Reservation.
11 - Show Strong Sad the Fake Sword at Homesar's Reservation.
12 - Talk to Strong Sad at Homestar's House.
13 - Talk to Strong Sad in Extended Mode

Well, I guess someone beat me to it. Just finished cataloging all insults plus other collectibles. Turned out it was the second use of the fake sword that eluded me. Never would have thought of that, sneaky telltale game designers:p.

It's entirely possible you're not paying much attention, seeing as how I gave exact instructions for all 13 just two posts above you.
it's quite easy to overlook, when you do the pom pom area before the reservation...meh
...what is far worse, is that i found all flags in my first playthrough and in the second can't find two of them anymore.:o
