I agree with HSP regarding the hallucination - it's a cliche. Maybe you could change it into Clem asking herself what Lee would do, remembering his words and decisions? It's just a suggestion.
I totally agree with TheMissus. It makes more sense because she hasn't shown any other signs of hallucinations in the past, making this feel out of place.

I... don't know what to write, actually. That thing with the men threatening Clem is somewhat out of place and seems forced. Never believed I would say that right at the end of the season, but: I'm out of ideas for both storylines. Maybe Moto or HSP can make something out of it?

Clem almost didn't register the identity of the walker despite knowing who he was as she just kicked him off and ran in the opposite direction.
"Hey, Girl!!" A Male voice that likely belonged to whoever shot the walker called out as she continued to run, but she didn't stop, She just kept running

"Uh...I-I'm...." Clem started to say, but she stopped, not sure if she should introduce herself to the stranger.
"...Yeeeees?" The girl asked, still smiling, Shortly after a bark was heard in the woods and a small Pug rushed out, the Girl knelt down and picked the pug up, before standing up again. "There you are..." She smiled to the pug. Before quickly snapping back to Clementine. The girl looked harmless, she didn't even seemed to be armed, but her backpack was filled to the brim with all kinds of stuff. Her ruffled Fair Hair was held back by a small dragonfly clip and she had a small beauty spot under her cheek.
"Oh, maybe you know what those screams were." The girl commented,randomly.

"What should I do...?" The thoughts go through Clementine's head, spinning around, and around. If I go back, I could die... But if I stay here… more then I'll never see them again.. Her thoughts are interrupted by the sound of walkers. "Shit!" Clementine says, as she looks in front of her.
A massive amount of walkers stand in front of her. "No..." Clementine says, doubtful if she will live any longer. With few weapons to protect herself, Clementine runs forward, trying to get away. She had begun to get surrounded, and shoots the last 9 bullets in her pistol. She then trys running to get away.
She had been running so fast, looking back to see the whereabouts of the other walkers when she runs straight into a tree, and gets knocked down. Felling sick, and terrible, she couldn't get herself to stand up. She grabs her hunting knife out of her pocket, and gets it ready.

I honestly don't know about introducing new characters now, maybe she quickly gets away from the story though or something like that, in which case is somewhat fine.

The little girl stands there smiling. That's kind of creepy... Clementine thinks.
When all of a sudden Kenny comes rushing from behind a tree, breathing heavily. "I'm Huff I saved you back there Clem..." He says before huffing one more time. "And who the heck is this...?" Clementine gives him a look, as if she doesn't know. "I don't know.. who are you?" Clementine asks, turning back to her.
"My name is Thalia. Well it's actually my middle name." Clementine nods and begins to speak. "We have to get out of here! There's way to many walkers out here.. We have to go now!" Right as Clementine's words come out, a walker buries it's teeth into Thalia. She screams, as it pulls her arm off. Clementine, wasn't quick enough, and stabs it with her knife.
The girls now lifeless body lies on the ground, the Pug lays alive on the ground know, un-bitten. Clementine picks it up, and reads it's nametag. "Sandy... That's her name.. Sandy" Clementine says. "That's what we're going to call it.." Kenny laughs, trying to brighten the mood. "It reminds me of Miranda."
Right as the two begin to walk away, Clementine notices something on Kenny's shoulder. It's a large walker bite. "Kenny...?"

After Clementine has cried all her tears Kenny took a deep breath, and spoke again. "Clem, you didn't kill Lee, and you didn't kill me. What happened to us was our own god damn faults. We came to get you because we wanted to, and we'd do it again. You didn't force us to do anything sweet pee, we came after you because we wanted to, because we love you, and lastly because you became like a daughter to both of us." Kenny's eyes where watering by the time he was finished.
Clementine still didn't feel much better, but she knew Kenny was telling the truth. Still she felt like it was her fault. "What do we do now ?" It was funny how many times she had asked that question.
"I may not like the guy but, Bryan knows what he's doing, and he really cares for you. I got to get you to him before... you know."

Kenny points to the large chunk of flesh missing from his shoulder. A mix of sadness, anger and a slight hint of nausea fills Clementine's mind. Pulling out her hunting knife, she stands up and tells Kenny to follow her closely. With her back straightened and her head held up high, the two begin to walk through the forest as silently as possible, making sure the dog follows. ''This is where I came from, the others should be around here somewhere.'' Kenny points to a small clearing. She shakes her head, ''Can't you see? It's full of walkers.'' Her confidence shrinks as she desperately tries to think of their next move. She looks around when something strikes her as familiar. Running away from him, Kenny loudly whispers to her to come back. A bolt of adrenaline rushes through her veins when she finds the small hatch leading to Bryan's cave. ''Kenny!'' she shouts with newly found hope. The middle-aged man struggles to catch his breath when he finally reaches her. He watches as she opens the heavy entrance. She tells him to go first, but he refuses when the growls of flesh-longing walkers approach them. Just in time, Clementine closes the hatch, leaving them alone in the dark. A sigh of relief escapes from her mouth when Kenny hands her a small lighter, turning it on instantly. ''Better than nothing.'' he mumbles, pointing her to the small room at the end of the corridor. Memories of the night they had spend in this shelter come rushing back like a freight-train, moistening her eyes once again. Quietly sobbing, she wipes her tears away.

''Come on, Kenny, just lay down right there. I'll make a fire to keep us warm.'' Struggling to hold back her tears Clementine lights the round campfire in the middle of the cave. The warmth of the flames comforts her, yet Kenny's miserable state leaves her feeling empty and alone. ''Kenny?'' she says with a cracking voice. ''It's just like with Lee, and even Duck, isn't it?'' As he slowly nods his head, a tear rolls down his pale face. Her heart broken once again, Clementine's thick shell breaks as she finally lets out the tears she's been holding back since the moment they had reunited. She rushes to her protector, holding his weak body tightly. ''I'll miss you...'' she says while standing back from Kenny. ''I'll miss you too...'' he points to the gun in his pocket. The flow of tears grows larger when the realization of what she's about to do strikes. Unwillingly, Clementine picks up the pistol with trembling hands. ''I don't want to do this again...'' Waiting for a response, she desperately stares at Kenny's lifeless eyes. His chest has stopped moving, his arms lay still. Straightening her back, holding her head up high, Clementine determinedly aims her gun at her former protector, pulling the trigger while saying her final goodbyes.

Love it. I hope this save our story. We've never had so much chaos but I am positive that this saves the ending.

''Come on, Kenny, just lay down right there. I'll make a fire to keep us warm.'' Struggling to hold back her tears Clementine lights the round campfire in the middle of the cave. The warmth of the flames comforts her, yet Kenny's miserable state leaves her feeling empty and alone. ''Kenny?'' she says with a cracking voice. ''It's just like with Lee, and even Duck, isn't it?'' As he slowly nods his head, a tear rolls down his pale face. Her heart broken once again, Clementine's thick shell breaks as she finally lets out the tears she's been holding back since the moment they had reunited. She rushes to her protector, holding his weak body tightly. ''I'll miss you...'' she says while standing back from Kenny. ''I'll miss you too...'' he points to the gun in his pocket. The flow of tears grows larger when the realization of what she's about to do strikes. Unwillingly, Clementine picks up the pistol with trembling hands. ''I don't want to do this again...'' Waiting for a response, she desperately stares at Kenny's lifeless eyes. His chest has stopped moving, his arms lay still. Straightening her back, holding her head up high, Clementine determinedly aims her gun at her former protector, pulling the trigger while saying her final goodbyes.
''Goodbye, Kenny.''
