I can't take an admission form, in Episode 101

please help me.
I reached front of Brady Culture's House and clicked on ADMISSION FORM,
but SAM didn't take an admission form, so I can't go inside.
I play PC version. Please help me.

    Uh... Check your inventory. If it's not there try again... yeah that's all I got.

    Okay, first things first: try talking with Sybil first (hey, she's a psychothereapist. Why the heck not?). Then you might be able to get the form. If that doesn't work, check your inventory. If all else fails, best go to the walkthroughs.
