Did you save Duck at the farm, defend him at the drugstore, give him something to eat, both at the drugstore and at the motel, did you help kill Larry, did you tell him about your past, etc. I'm trying to figure out what you did that stopped him from going with you.

I just want to chime in here; the specific reason (in terms of Kenny's coding within the game) for Kenny not coming with you is the fact that you made him kill Duck. While as far as the story is concerned, he is grateful towards you for it, the decision-making code that determines his disposition towards the player, is not. While I can see a lot of sense in why someone would have him shoot his son, and while there are fully justifiable and ethical reasonings to this decision, the game itself considers this to be the objectively "bad" choice out of the two options presented, which is what ultimately causes him to not go with you after this choice is fed through the "machine", so to speak.
One thing that interested me while watching other playthroughs and reading posts is the somewhat erratic behavior of Kenny; not so much the behavior specifically tied to his character, but the behavior that would be tied to his "mind", or rather, what the game considered pro-Kenny/anti-Kenny. I've been doing multiple playthroughs on my own on top of watching other playthroughs in order to try and pinpoint the specific key decisions that actively determine his disposition towards the player, specifically those of the "who came with you" choice.
There are five key decisions that directly determine his "choice" to come with you or not:
Who you save on Hershel's farm (Save Duck)
Who you side with in the drug store (Side with Kenny)
Whether or not Duck is fed in Episode 2 (Feed him)
Helping him kill Larry or not (Help him)
Shooting Duck or making him do it himself (Shoot Duck yourself)
However, whether it's truly intended behavior for Kenny's decision, the game seems to work on a "5 or bust" system; if you don't meet even one of the above criteria, the game will interpret that as being against Kenny, which will ultimately make him give the infamous "and I am a Christian man" spiel to you. It doesn't matter which specific one isn't met, as long as one of those above 5 are not met, it'll produce the same results. The reason I don't personally believe that this is the intended behavior is because of how the dialogue and presentation is set up. The speech he gives would make sense if you, say, hit 2 or 3 of the 5 choices listed, but the fact that one missing one can essentially fully flip his character's opinion on Lee entirely seem like it'd be a fault of the way his character was coded, rather than him just being an ungrateful dipshit. While Kenny's character is undoubtedly a very divisive and polarizing character, this scene felt "wrong" to me on a mechanical/technical level.
Ben's fate, the Lilly vs. Kenny debates, the boy in the attic, whether you feed him specifically in Episode 2, and the train scene all don't have an active role in determining this specific choice; they DO effect his opinion of Lee on other decisions, dialogues, or arguments, but they don't play an active role in the Clementine decision in particular.
To be fully honest, I'd almost love to just chat with the programming team about this whole thing; it's fairly interesting trying to pick this apart.
Unfortunately, we have to take his character the way it is coded, as it is nothing more than code to begin with.
