I remember people spotting the spooky face in the key art around the time it was first released :P
It's not the ghost of Lee and it was not intentional but it is a phenomenon called "pareidolia" where humans are able to see face like patterns where none exist.
Nobody. It's just a badly positioned selectable. The first episode has a ton of badly placed selectables. Try playing episode 1 with them off. Unless you know exactly where they are, it's pretty annoying at times.
