Duck and Sarah made perfect sense, they weren't good survivors but they were in protective groups. Also dialogue suggests they hid or repressed a lot of what had happened to them.
That said I do think most people alive should be fairly capable by this point and dont do stuff that is just dumb for an experienced survivor

I guess you could stretch it to that, in my opinion that scene is either clementine manipulating sarah or sarah manipulating clementine into being her friend (she insists you be her friend before she gives it to you or if you wont she gives it to you so she doesnt get in trouble), not to help clementine. Clementines face is that of hurry and bewilderment at sarahs sudden desire to be friends (and everything else she does), she can become her friend over time but in that first scene clementine just appears weirded out.
Regardless, im sure the medicine helped but Carlos looked at it later and likely helped with it.
