Kate is Alive.
Before the Molotov hits the car, you can clearly see Kate in the same spot as the image below.
After the Molotov hits the car, and the last shot we see of the front seat the second before it crashes. As you can see we can only see an empty seat and the tip of the steering wheel, No Kate.
Kate is alive, she must have rolled out of the car before the explosion or found some way to get out.

In all seriousness, I am certain that she is alive. This to me seems like they're setting her up, just like Glenn and the Dumpster scene, for something bigger between Javier and David.

I hope yall realise that if she rolled out of the car, we would have seen her right in front of us. She's dead. And if by some chance she's still alive..the 1000 walkers entering Richmond will take care of that.

We didn't see the body, so she's (somehow) alive. I know those trucks are heavily armored, but getting hit by a molotov cocktail, then crashing into a tank truck and causing that big of an explosion, she should be dead. But we all know Telltale isn't going to do that, she'll show up eventually in Episode 5.
