Definitely Dee's death in 400 days. I just found that so gruesome and sickening, how you accidentally hit her and she's lying on the ground with a fractured skull and busted eye with blood all over her face. I thought that was horrifying, especially since I did it un-intentionally, and she just lays there blaming me for killing her while I can't do anything.
There have been plenty of gruesome moments on all three mediums so i will put one from each category below.
Game: Carver getting his face smashed in with the crowbar.
Comic: Glenn's death.
TV Show: Noah's death in the revolving door.
Kenny's epic killing of Larry (which I happily assisted)
Brie's death
Lee's walker killing spree
Lee getting his arm cut off and Clementine sowing her arm back toghether
For the characters themselves, I imagine it would be knowing that you and everyone around you is most likely doomed.
I often find the Comic the goriest of the three, and they can't even use red to highlight blood. Adlard does a great job in making scenes seem brutal.
