Unfortunately, I've never had the pleasure of playing the games but just watching them. For those who have played, I wonder if you regretted your first decisions in retrospect when playing the first time. If so, can you recall what they were?

Off the top of my head, the first choice was simply telling Kenny to stop shooting at Carver.
The thing is when I was playing Season Two for the first time, I usually took at least a one day break between episodes. This habit unfortunately meant I forgot to rewind and change this choice in particular in part because it was technically the final choice of the episode. In other words, I first played the next episode with Alvin already dead, which is a serious break and no-no from how I would normally play these games.
I think I regretted not taking Clementine with us to Crawford that night. Really hard for me to remember this season one choice in particular because it was so long ago and the first time I had played and also wasn't significant in effects. But I think I made every choice with parental logic and Clem's best intestest in mind except agreeing to give her up to Vernon/Stranger. Really seemed like either decision could be dangerous but leaving her with Omid felt less dangerous at the time. EDIT: Now that I think about it I believe I DID take Clementine..because she ended up saving Molly and I remember her little cute speech about how we don't leave friends behind etc.... then she leaves Sarah behind next season.LOL good times. Well since I couldn't remember this choice my next biggest S1 regret was not leaving Lilly on the road after killing Doug by accident.
Second biggest regret in all of my history of playing telltales games was in season two.. where I unawaringly regretted not leaving Kenny at the gates of Wellington entrance due to his then abominable written future death ? that will take place in season three.
In season three I regretted taking Clem's side with Eli because it wasn't right and it was just my protective instinct and loyalty to Clem that made my choice skewed in her favor. Same with the Conrad subway ? dilemma. I also regretted not shooting Joan thinking she would somehow be saved before the bullet would get to her or I'd be stopped as Javi.
Ever have a choice so difficult to make you had to pause the game?
After thinking about it a bit, no, but there are definitely plenty of choices where I nearly let the timer run out trying to decide. Not always successfully.Sorry Ben.

The choice in GoT where you had to save one of the brothers at the end of episode 4 and in TWD ANF i paused between Tripp and Ava
Almost all of the choices because I'm bad at making choices. Seriously, I'm bad.
The only choice i regret was going with Jane in season 2 because of how bad they treated her in A New Frontier.

So..Yeah, if we're also talking about pausing the game because a choice is too difficult i did that once.
In Season 1 Episode 2,in the meat locker,that choice was really hard.
This is what they call "Rage-Pausers" you pause the game,take a walk around the house because you can't decide what you want to do.
Would you call that "cheating" if you did pause it....because that allows far more thinking ? time than is intended by the game for a critical decision?
Definitely Shooting Kenny. Man, I really regret doing that. I dislike rewinding though because it ruins the experience for me personally.
I would say that counts as cheating, along with rewinding to change your decisions.
The hardest choice for me by a mile was either staying in Wellington or going with Kenny. I know there was no timer but i think i waited for like half an hour going over and over in my head what to do. Do i go with him or honour his last wish and think of AJ and Clem herself? It was so hard. I even rewinded just so i could read the dialogue leading up to it again in case i missed anything. The second i heard Kenny say “you’ll meet people who won’t have to look at you and feel ashamed at what they’ve put you through”, I knew then I couldn’t leave him.
I've watch a lot of gameplays and never saw anyone leave Clem with Omid. What happened?
I think it's a bit cheating. The game gives you a certain amount of time to do things and then it reacts for you. Pausing doesn't let the game proceed naturally. That being said, I can understand it though.
