How did Lilly not recognize Clementine's hat?
She only recognize's here by looking at here face. Wasn't the hat obvious enough?

The first time Lilly sees Clem is with Clem on her back. For Lilly it was just a blue hat.
Besides, it's not like this hat is unique. There are probably hunders of thousands of those laying on someone elses had or catching dust in the ruined clothes store.

However AJ calling out Clem's full name, plus the hat, makes it a safe bet that it's the same Clementine she knew in the beginning. Clementine isn't a common name and the hat, while sure, being a mass produced item, is still probably not very common to see given all the other millions of hats that would be available.
Lilly recognized clem from Aj saying her name and the hat. Just pay attention to the camera, the devs made sure the hat was right in front of the camera before clem looks up.
Most people’s own aunts or uncles can barely recognize their relatives when the last time they saw em was as a child. Teachers too.
