What the fuck happened to the rest of Shel's group (cancer patients)? - Clem, Pete and Nick find the leader's body at the river. I assume the cancer patients either fled after dude went hunting for Shel/Becca or dude killed them for fear or subordination.
What the fuck happened to Nate? - Don't remember if Wyatt or Russell story goes last but his character was not a fit for this season.
What the fuck happened to Leland? - I assume he is killed by Shel's group for his woman stealing from them.
Where the fuck is Lilly? She's dead, or with other group.

Kenny accused her of something before even talking and got fired up because she could start talking. IDK if she did think he would try to kill her, maybe battle enough for Clem to not want to go with him.
My opinion (some may be similar to BeautifulBanjo's). This is all speculation. If there are contradictions then I simply forgot some details.

It is possible she died maybe from starvation or getting shot, she may have found another group (unlikely).

I presumed it was a miscarriage, It may have been born and died later.
